Please pay special attention to the Special Requests section, as these are the only requests we honor for playing on specific teams.

New youth players: submit a copy of the player's birth certificate via email, fax, or bringing it to the office. New adult players: Have your picture taken at the office for a player's card. Bring your driver's license for verification.
Review the Parent Code of Conduct
Your coach will contact you regarding practice times, team name, uniforms, and the medical release form once they receive their roster, a few weeks before the season begins.
